Key Responsibilities:

• Perform thorough annual or post-discharge home health risk assessments in patient homes throughout Payette, Fruitland, New Plymouth, and surrounding areas
• Review and evaluate medical, surgical, and medication histories
• Educate patients on current healthcare conditions and appropriate medication use
• Identify gaps in care, offering clear recommendations for next steps
• No requirements for developing treatment plans or prescribing medications

Qualifications:

• Active Nurse Practitioner license in Idaho
• MSN degree from an accredited nursing program
• Board Certification through AANP or ANCC
• Current CPR (BLS) certification
• At least 6 months of recent Nurse Practitioner experience preferred
