Schedule:
- Flexible shifts available Monday through Sunday
- Design your own work schedule
- Commit to either weekly or bi-weekly minimum hours
Key Responsibilities:
- Complete annual home health risk assessments for Medicare Advantage plan members across Payette County
- Conduct comprehensive reviews of medical histories, existing conditions, and medication usage
- Deliver clear education to patients about their health conditions and prescribed medications
- Pinpoint any care gaps, offering recommendations to improve health outcomes
- No requirement to develop active treatment plans or issue prescriptions
Qualifications:
- Licensed as a Nurse Practitioner in Idaho
- Current CPR certification (BLS)
- Board certification via AANP or ANCC
- At least 6 months of recent NP experience preferred
- MSN degree from an accredited nursing program
