Job Description

Make a meaningful impact in community health as a Nurse Practitioner providing home health risk assessments to Medicare Advantage members throughout Payette County. In this per diem role, youll enjoy the flexibility to tailor your own work schedule while serving residents in areas including Payette, Fruitland, and New Plymouth.

Schedule:

  • Flexible shifts between Monday and Sunday
  • Set your own schedule
  • Minimum weekly or bi-weekly hourly commitment

Key Responsibilities:

  • Perform in-home annual health risk assessments for Medicare Advantage members
  • Review and evaluate medical history, ongoing conditions, and medication usage
  • Offer education and guidance about medical conditions and medications
  • Identify and document gaps in care, recommending steps for improvement
  • Visits may involve annual assessments or post-discharge evaluations
  • No requirement to create treatment plans or prescribe medications

Qualifications:

  • Active Nurse Practitioner license in Idaho
  • MSN from an accredited nursing program
  • Current BLS (CPR) certification
  • Board certification through AANP or ANCC
  • At least six months of recent Nurse Practitioner experience preferred
