A dynamic opportunity is available for a Nurse Practitioner skilled in home health assessments. This per diem position is designed for providers who thrive on flexibility and value impactful patient care in a home setting. You will conduct comprehensive risk assessments for Medicare Advantage plan members located throughout Providence County, Rhode Island, including cities such as Woonsocket, Scituate, and Cranston.
Schedule/Hours
- Flexible shifts available between Monday and Sunday
- Create your own schedule based on your availability
- Commitment can be on a weekly or bi-weekly hourly basis
Key Responsibilities
- Perform annual home health risk assessments, each typically taking about 45 minutes
- Review and evaluate patients prior and current medical history, conditions, and medication profiles
- Provide personalized education regarding health conditions and medication usage
- Identify and document care gaps, offering actionable recommendations for closing those gaps
- Note: No treatment planning or medication prescribing required
Qualifications
- Active Nurse Practitioner License in Rhode Island
- Board Certification through AANP or ANCC
- Current CPR (BLS) certification required
- Minimum of 6 months' experience as a nurse practitioner preferred
- MSN degree from an accredited institution
