Job Description
Take on a rewarding and autonomous role providing direct care to Medicare Advantage plan members in Minidoka County. As a Nurse Practitioner specializing in home health, you will carry out risk assessments for patients in their homes, supporting preventative care and promoting well-being across locations such as Rupert, Heyburn, and Paul.
Schedule:
- Flexible shifts available Monday through Sunday
- Set your own work schedule
- Minimum weekly or bi-weekly hourly commitment required
Key Responsibilities:
- Perform comprehensive annual health risk assessments during home visits, with each assessment typically lasting about 45 minutes
- Review and evaluate prior and current medical histories, medical conditions, and medication lists
- Educate members about their conditions and prescribed medications
- Identify potential gaps in care and offer actionable recommendations for improvement
- Focus exclusively on assessment; no treatment planning or prescription-writing necessary
Desired Qualifications:
- Valid Nurse Practitioner License in Idaho
- Active CPR certification (BLS)
- Board Certification via AANP or ANCC
- At least 6 months of recent Nurse Practitioner experience preferred
- Master of Science in Nursing (MSN) from an accredited school
This per diem opportunity offers a highly competitive compensation of $80-$90 for each evaluation you complete, along with additional mileage reimbursement and payment for administrative time.
