Job Description

Take on a rewarding and autonomous role providing direct care to Medicare Advantage plan members in Minidoka County. As a Nurse Practitioner specializing in home health, you will carry out risk assessments for patients in their homes, supporting preventative care and promoting well-being across locations such as Rupert, Heyburn, and Paul.

Schedule:

  • Flexible shifts available Monday through Sunday
  • Set your own work schedule
  • Minimum weekly or bi-weekly hourly commitment required

Key Responsibilities:

  • Perform comprehensive annual health risk assessments during home visits, with each assessment typically lasting about 45 minutes
  • Review and evaluate prior and current medical histories, medical conditions, and medication lists
  • Educate members about their conditions and prescribed medications
  • Identify potential gaps in care and offer actionable recommendations for improvement
  • Focus exclusively on assessment; no treatment planning or prescription-writing necessary

Desired Qualifications:

  • Valid Nurse Practitioner License in Idaho
  • Active CPR certification (BLS)
  • Board Certification via AANP or ANCC
  • At least 6 months of recent Nurse Practitioner experience preferred
  • Master of Science in Nursing (MSN) from an accredited school

This per diem opportunity offers a highly competitive compensation of $80-$90 for each evaluation you complete, along with additional mileage reimbursement and payment for administrative time.
