This per diem opportunity puts you at the forefront of proactive healthcare, focusing on annual assessments and post-discharge visits without responsibilities for ongoing treatment planning or prescribing. Schedule:

  • Flexible shifts available Monday through Sunday
  • Design your own shifts and weekly commitment
  • Minimum hourly commitment on a weekly or bi-weekly basis

Primary Responsibilities:

  • Conduct comprehensive annual health risk assessments in patient homes
  • Evaluate both past and current health conditions, including all medications
  • Educate patients about their diagnoses and therapies
  • Identify and communicate gaps in care, recommending steps to address them
  • Perform visits in various locations across Cassia County, including Burley, Oakley, and Declo

Desired Qualifications:

  • Active Nurse Practitioner license in Idaho
  • Current BLS (CPR) certification
  • Board certified via AANP or ANCC
  • MSN from an accredited nursing school
  • Minimum of six months recent NP experience preferred
