Home health nurse practitioners are needed to conduct in-home risk assessments for Medicare Advantage plan members throughout Chesterfield County, SC. Typical visits last around 45 minutes and may include comprehensive annual health assessments or post-discharge evaluations. Each shift may take you to various areas within Chesterfield County, such as Cheraw, Chesterfield, or Pageland.

Schedule:

• Flexible shifts offered Monday through Sunday
• Design your own work schedule
• Minimal weekly or bi-weekly hourly commitment required

Responsibilities:

• Complete thorough in-home health risk assessments for Medicare Advantage members
• Review and document prior and current medical history as well as medication use
• Deliver patient education regarding diagnoses and prescribed medications
• Identify care gaps and recommend practical solutions to improve outcomes
• No need to develop treatment plans or prescribe medications

Qualifications:

• Current Nurse Practitioner license in South Carolina
• BLS (CPR) certification required
• AANP or ANCC board certification
• Minimum six months of recent experience as a nurse practitioner preferred
• MSN degree from an accredited nursing program

This position offers compensation of $80$90 per evaluation conducted, plus a mileage stipend and payment for administrative time.
