You will play a critical role by conducting risk assessments for Medicare Advantage members residing in Hudson County, including areas such as Jersey City, Bayonne, and Union City. This position is ideal for Nurse Practitioners who enjoy autonomy, variety in their day, and the ability to help patients where they feel most comfortabletheir homes.
Schedule:
- Flexible shifts available Monday through Sunday
- Design your own work schedule
- Commitment to a minimum of weekly or bi-weekly hours
Primary Responsibilities:
- Perform comprehensive annual home health risk assessments for Medicare Advantage plan members
- Assess medical histories, existing conditions, and current medications
- Educate members about their health conditions and medications
- Identify gaps in care and recommend appropriate next steps
- No requirement to develop treatment plans or prescribe medications
Key Qualifications:
- Active Nurse Practitioner license in New Jersey
- Valid CPR certification (BLS)
- Board certification from AANP or ANCC
- At least 6 months of recent NP experience preferred
- MSN from an accredited nursing school
Compensation for this role ranges from $80$90 per completed evaluation, and includes both a mileage stipend and consideration for administrative time.
