Looking for an independent advanced practice nursing position with flexibility and variety? This per diem opportunity offers the chance to perform comprehensive home health risk assessments for Medicare Advantage members throughout Morris County, NJ. Nurse Practitioners will visit patients in locations including Parsippany-Troy Hills, Mount Olive, and Randolph, with each assessment taking approximately 45 minutes. Visits may focus on annual evaluations or post-discharge needs, and your expertise will help identify gaps in care, improve patient understanding, and connect members to additional resources.
Schedule:
- Flexible shifts available Monday through Sunday
- Self-scheduling to match your availability
- Minimum weekly or bi-weekly commitment required
Key Responsibilities:
- Conduct in-person annual and post-discharge home health risk assessments in patient residences
- Review prior and current medical history, conditions, and medication regimens
- Deliver education on chronic conditions and medication management to members
- Identify gaps in care and recommend appropriate follow-up, improving overall healthcare outcomes
- No on-call duties, prescription writing, or treatment plan development required
Desired Qualifications:
- Active Nurse Practitioner license in New Jersey
- Board Certification through AANP or ANCC
- Current BLS (CPR) certification
- Master of Science in Nursing (MSN) from an accredited program
- Minimum 6 months recent NP experience preferred
