Seeking a skilled Nurse Practitioner to perform home health assessments in a per diem capacity. This important role involves visiting Medicare Advantage members in their homes throughout Union County, providing comprehensive risk assessments that inform ongoing care.

Schedule:

• Flexible shifts available Monday through Sunday
• Create your own schedule to suit your availability
• Choose a minimum commitment for weekly or bi-weekly hours

Key Responsibilities:

• Conduct thorough annual risk assessments for Medicare Advantage health plan members in areas such as Elizabeth, Plainfield, and Linden
• Review and evaluate each patients medical history, current conditions, and medication regimen during each 45-minute home visit
• Educate patients on their health conditions and medications, promoting informed participation in their care
• Identify gaps in care and offer actionable recommendations to optimize patient health, without the need for prescribing medications or developing treatment plans
• Complete administrative documentation for each assessment

Qualifications:

• Active Nurse Practitioner license in New Jersey
• Board Certification through AANP or ANCC
• Masters of Science in Nursing (MSN) from an accredited institution
• At least 6 months of recent NP experience preferred
• Current Basic Life Support (BLS) certification

This opportunity provides highly competitive compensation, with $80 to $90 per completed evaluation, along with a mileage stipend and compensation for administrative tasks.
