Job Description

Expand your impact in patient care by providing home-based health risk assessments for Medicare Advantage members in Morris County, NJ. This per diem opportunity offers a flexible schedule and the ability to create your own shifts, accommodating both weekday and weekend availability. You will perform visits in various locations within Morris County, including Parsippany-Troy Hills, Mount Olive, Randolph, and surrounding areas, making a difference for patients in their own homes.

Schedule:

  • Flexible shifts between Monday and Sunday
  • Create your own schedule
  • Minimum weekly or bi-weekly hourly commitment

Key Responsibilities:

  • Conduct annual home health risk assessments for Medicare Advantage plan members in their residences
  • Review prior and current medical history, ongoing health conditions, and medication lists
  • Offer patient education regarding medical diagnoses and prescribed medications
  • Identify care gaps and suggest actionable recommendations for improved health outcomes
  • Focus exclusively on assessmentsno requirement to prescribe or create treatment plans

Qualifications:

  • Valid Nurse Practitioner license in NJ
  • Active Board Certification through AANP or ANCC
  • Current BLS (CPR) certification
  • Master of Science in Nursing (MSN) from an accredited institution
  • Minimum of 6 months of current NP experience preferred, but not required
