This per diem position offers unmatched flexibility, allowing you to shape your work schedule around your lifestyle with shifts available Monday through Sunday. Each home visit typically takes about 45 minutes and may involve annual health assessments or post-discharge evaluations for members residing in Jersey City, Bayonne, Union City, and throughout Hudson County, NJ. Youll receive competitive compensation for each evaluation, along with a mileage stipend and paid administrative time.
Schedule:
- Flexible shifts available Monday through Sunday
- Design your own work schedule
- Meet minimum weekly or bi-weekly hourly commitments
Key Responsibilities:
- Perform thorough annual home health risk assessments for Medicare Advantage health plan participants
- Review prior and current medical histories, conditions, and medication regimens
- Deliver patient education regarding medical issues and medications
- Recognize gaps in care and recommend suitable follow-up steps
- Note: This role does not require the creation of treatment plans or prescription of medications
Qualifications:
- Active NJ Nurse Practitioner license
- Board certified through AANP or ANCC
- Graduation from an accredited MSN program
- Current CPR (BLS) certification
- Minimum of six months recent Nurse Practitioner experience preferred
