Seeking an experienced Nurse Practitioner to perform home health risk assessments for Medicare Advantage members throughout Pasco and Franklin County, WAincluding communities such as Connell, Kahlotus, and Mesa. This per diem opportunity offers unmatched flexibility, allowing you to set your own shifts and align your work schedule with your availability. Each visit is focused, averaging 45 minutes, and may range from annual assessments to post-discharge evaluations.

Schedule:

• Choose your own shifts between Monday and Sunday
• Flexible scheduling to meet your needs
• Minimum commitment required weekly or bi-weekly

Key Responsibilities:

• Conduct comprehensive annual home health risk assessments for Medicare Advantage plan members in their homes
• Review patients prior and current medical history, conditions, and medications
• Provide education to members about their conditions and prescribed medications
• Identify gaps in care and offer actionable recommendations
• No responsibility for creating treatment plans or prescribing medications

Qualifications:

• Active Nurse Practitioner license in Washington (WA)
• Board Certification through AANP or ANCC
• Current CPR certification (BLS)
• MSN degree from an accredited nursing school
• Minimum 6 months experience as a Nurse Practitioner preferred, though all levels are encouraged to apply

As part of this independent and rewarding position, compensation is provided at $80-$90 per completed evaluation, with an additional mileage stipend as well as compensation for administrative time.
