Schedule:

  • Flexible shifts available any day, Monday through Sunday
  • Self-scheduling to match your availability
  • Minimum weekly or bi-weekly hourly commitments ensure balance

Key Responsibilities:

  • Perform comprehensive home health risk assessments, each lasting approximately 45 minutes
  • Evaluate patients prior and current medical history, conditions, and medication usage
  • Educate patients about their health conditions and prescribed medications
  • Detect and report any gaps in care, recommending practical solutions
  • Focus on thorough evaluations without needing to create treatment plans or prescribe medications

Qualifications:

  • Active Nurse Practitioner license in Tennessee
  • Current CPR (BLS) certification
  • Board certification via AANP or ANCC
  • At least six months of recent Nurse Practitioner experience preferred
  • Master of Science in Nursing (MSN) from an accredited institution
