Advance your clinical practice as a Nurse Practitioner working in home health risk assessments. This per diem role is essential in ensuring quality care for Medicare Advantage members in Henderson County, Tennessee. You will perform thorough evaluations for patients in locations including Lexington, Parkers Crossroads, and Sardis. Each home visit averages 45 minutes and can include annual health assessments or post-discharge visits, depending on patient needs.
Schedule:
- Flexible shifts available seven days a week
- Self-schedulingstructure your shifts as needed
- Maintain a minimum weekly or bi-weekly hours commitment
Primary Responsibilities:
- Complete annual home health risk assessments for Medicare Advantage plan members
- Review and evaluate past and current medical histories, conditions, and medications
- Educate patients on their diagnoses and proper medication usage
- Identify care gaps and make recommendations to address them
- No requirement for developing treatment plans or prescribing medications
Candidate Qualifications:
- Active Nurse Practitioner license in Tennessee
- Board certification through AANP or ANCC
- Current BLS (CPR) certification
- MSN degree from an accredited nursing program
- At least six months of Nurse Practitioner experience preferred
