Seeking an experienced Nurse Practitioner to perform comprehensive home health risk assessments on Medicare Advantage members throughout Payette County, ID. This per diem role offers unmatched flexibilitycreate your own schedule with shifts available any day between Monday and Sunday. Schedule

• Flexible shifts available Monday through Sunday
• Set your own hours with minimum weekly or bi-weekly commitments
• Assignments may occur throughout Payette County, including Payette, Fruitland, and New Plymouth

Responsibilities

• Conduct in-home annual and post-discharge health risk assessments for Medicare Advantage plan members
• Review and document medical history, chronic conditions, and current medications
• Educate patients on health conditions, risk factors, and medication adherence
• Identify and communicate care gaps while supplying recommendations for follow-up care
• No responsibility for treatment planning or prescribing medicationsfocus on assessments and patient education

Qualifications

• Active Nurse Practitioner license in Idaho
• Board Certification by AANP or ANCC
• Current CPR certification (BLS)
• Master of Science in Nursing (MSN) from an accredited institution
• At least 6 months of recent experience as a Nurse Practitioner preferred

You will earn $80$90 per evaluation, with additional compensation for mileage and administrative time, supporting your commitment as you travel throughout the county to provide high-quality care.
