Job Description

Nurse Practitioners with experience in home health are invited to provide essential care through flexible, per diem opportunities. In this role, you will perform comprehensive home health risk assessments for Medicare Advantage plan members throughout Payette County, including areas such as Payette, Fruitland, and New Plymouth. These visits are typically completed in approximately 45 minutes and may range from annual health assessments to post-discharge follow-ups.

Schedule:

  • Flexible shifts available throughout the week, Monday through Sunday
  • Ability to create your own schedule
  • Minimum weekly or bi-weekly hourly commitment required

Key Responsibilities:

  • Conduct in-depth annual home health risk assessments for Medicare Advantage members
  • Review and evaluate past and current medical history, existing conditions, and patient medication lists
  • Educate members about their health conditions and prescribed medications
  • Identify gaps in care and provide actionable recommendations to address them
  • No responsibility for creating treatment plans or prescribing medications

Qualifications:

  • Hold an active Nurse Practitioner license in Idaho
  • Current Basic Life Support (BLS/CPR) certification
  • Board certified through AANP or ANCC
  • Minimum of 6 months current NP experience preferred
  • MSN degree from an accredited nursing school
