Schedule/Hours:
- Flexible shifts between Monday and Sunday

Responsibilities:
- Conduct home health risk assessments in patient residences across Garfield County, which may include Glenwood Springs, Rifle, and Carbondale
- Perform evaluations lasting approximately 45 minutes to assess medical history, chronic conditions, and medications
- Complete both annual health assessments and post-discharge visits, as assigned
- Deliver education on diagnosed conditions and prescribed medications
- Identify and document gaps in care, recommending solutions for improvement
- Focus solely on assessment and educationno diagnosis, treatment planning, or prescribing required
Qualifications:
- Active Nurse Practitioner license in Colorado
- Current CPR certification (BLS)
- National board certification through AANP or ANCC
- Minimum of 6 months of current NP experience preferred
- MSN degree from an accredited nursing program
Nurse Practitioners in this per diem position receive competitive pay of $80$90 per evaluation, plus mileage reimbursement and compensation for administrative time.
