Seeking a Nurse Practitioner with home health experience to perform comprehensive risk assessments for Medicare Advantage members in Bay County, FL. This per diem position offers full schedule flexibility, allowing professionals to create their own workdays and hours, with shifts available between Monday and Sunday. Youll visit members in their homes throughout Panama City, Lynn Haven, Panama City Beach, and nearby communities, conducting evaluations that typically take about 45 minutes.
Schedule:

- Minimum weekly or bi-weekly commitment required
Key Responsibilities:
- Conduct annual home health risk assessments for Medicare Advantage plan members
- Evaluate medical history, chronic conditions, and medication usage during home visits
- Provide patient education regarding medical conditions and prescribed medications
- Identify care gaps and provide recommendations to address them
- Focus on assessments; no development of treatment plans or prescription writing required
Qualifications:
- Active Nurse Practitioner license in Florida
- Board certification by AANP or ANCC required
- Current BLS/CPR certification
- MSN degree from an accredited nursing program
- At least 6 months recent experience as a Nurse Practitioner preferred
- Home health care experience is highly valued
