Deliver essential healthcare services in the community as a Nurse Practitioner specializing in home health risk assessments. This per diem opportunity offers flexibility and autonomy, allowing you to create your own schedule and work across various locations within Florence County. Youll play a vital role in evaluating Medicare Advantage plan members during in-home visits, ensuring thorough and compassionate care.
Schedule:
- Flexible shifts available Monday through Sunday
- Set your own schedule
- Minimum weekly or bi-weekly hourly commitment required
Key Responsibilities:
- Perform annual and post-discharge home health risk assessments for Medicare Advantage members
- Review and evaluate a patients medical history, current conditions, and medication regimen during approximately 45-minute home visits
- Educate patients about their medical conditions and prescribed medications
- Identify care gaps and recommend solutions to optimize health outcomes
- No requirement to prescribe medications or develop ongoing treatment plans
Qualifications:
- Valid Nurse Practitioner License in South Carolina
- Current BLS (CPR) certification
- Board Certification through AANP or ANCC
- MSN degree from an accredited nursing program
- At least 6 months of experience working as an NP preferred
