Key Responsibilities:

• Perform annual and post-discharge home health risk assessments for Medicare Advantage members
• Review current and prior medical history, conditions, and medication regimens
• Offer education concerning conditions and medications to patients
• Identify care gaps and recommend actions to resolve them
• No requirement to create treatment plans or prescribe medications

Qualifications:

• Active Nurse Practitioner license in Washington
• Current CPR certification (BLS)
• Board Certification via AANP or ANCC
• Six months or more of NP experience preferred
• MSN degree from an accredited nursing program
