Advance your practice as a Nurse Practitioner in a flexible, per diem home health role focusing on risk assessments for Medicare Advantage members. Enjoy autonomy and create your own work schedule while making a valuable impact statewide. Youll conduct visits in patients homes in Palmdale, Lakeport, Buckhead Ridge, and throughout Glades County, FL.
Schedule:
- Flexible shifts available seven days a week
- Choose your own assignments and build a schedule that works for you
- Satisfy a minimum weekly or bi-weekly hourly commitment
Responsibilities:
- Complete annual and post-discharge home health risk assessments (approximately 45 minutes per visit) for Medicare Advantage plan members
- Review and evaluate patients prior and current medical history, health conditions, and medications
- Educate patients on their diagnoses and medications, helping them better manage their health
- Identify gaps in care, providing actionable recommendations for improvement
- Focus on high-quality assessment and educationno prescribing or development of treatment plans required
Qualifications:
- Active Nurse Practitioner license in Florida
- Board certified by AANP or ANCC
- Graduate of an accredited MSN nurse practitioner program
- Current BLS (CPR) certification
- At least 6 months of recent NP experience strongly preferred
