This unique opportunity allows nurse practitioners to engage directly with Medicare Advantage plan members throughout Flagler County, including Palm Coast, Flagler Beach, and Bunnell, supporting proactive healthcare in the comfort of patients homes.
Schedule:
- Flexible shifts available any day, Monday through Sunday
- Design your own work schedule with minimum weekly or bi-weekly hourly commitments
Key Responsibilities:
- Conduct comprehensive annual and post-discharge home health risk assessments for Medicare Advantage members
- Review and evaluate patients medical histories, current health status, and medication use
- Offer clear education to patients regarding their conditions and prescribed treatments
- Identify care gaps, providing insightful recommendationswithout the need for prescribing or developing treatment plans
Qualifications:
- Active Nurse Practitioner License in Florida
- Board Certification (AANP or ANCC) required
- Current BLS (CPR) certification
- MSN from an accredited program
- At least 6 months of nurse practitioner experience (strongly preferred)
