Deliver vital care in the community by completing home health risk assessments as a Nurse Practitioner in the Palm Coast area. This per diem opportunity focuses on providing high-quality, patient-centric evaluations for Medicare Advantage plan members residing throughout Flagler County, including Palm Coast, Flagler Beach, and Bunnell. Schedule:
- Flexible shifts available Monday through Sunday
- Autonomy to create your own schedule
- Minimum weekly or bi-weekly hourly commitment based on availability
Key Responsibilities:
- Conduct annual home health risk assessments for members of a Medicare Advantage health plan
- Travel to patient residences in assigned regions within Flagler County
- Review prior/current medical history, current diagnoses, and medication routines
- Deliver patient education on health conditions and medications
- Identify care gaps and offer actionable recommendations to promote better health outcomes
- Document findings, but no treatment planning or prescribing is required
Qualifications:
- Active Nurse Practitioner license in the state of Florida
- CPR certification (BLS)
- Board Certification through the AANP or ANCC
- Minimum of six months' experience as a Nurse Practitioner (preferred)
- MSN degree from an accredited nursing program
