Deliver vital patient-centered care while enjoying the autonomy and flexibility of per diem work as a Nurse Practitioner conducting home health risk assessments. This role focuses on supporting the health and well-being of Medicare Advantage plan members throughout Mesa County, including Grand Junction, Fruita, and Palisade. Assessments average 45 minutes each and vary from annual health checkups to post-discharge evaluations. You will travel to member homes within the designated region and have the freedom to craft your own work schedule.
Schedule:
- Flexible shifts available any day, Monday through Sunday
- Design your own schedule to suit your availability
- Meet a minimal weekly or bi-weekly hourly commitment
Key Responsibilities:
- Perform comprehensive home health risk assessments for Medicare Advantage members
- Review patient histories, current health status, and medication regimens
- Offer education about medical conditions and prescribed medications
- Identify care gaps and propose actionable recommendations
- No need to create treatment plans or prescribe medications
Qualifications:
- Active Nurse Practitioner License in Colorado
- BLS certification (current CPR)
- Board Certification through AANP or ANCC
- Preferably at least 6 months of Nurse Practitioner experience
- MSN from an accredited nursing program
Compensation for this opportunity is $80$90 per evaluation, with mileage reimbursement and paid administrative time included.
