Seeking a dedicated Nurse Practitioner with home health experience to perform risk assessments in Mesa County, CO. This per diem position offers flexible scheduling and the autonomy to create your own shifts, with opportunities to serve diverse communities within Grand Junction, Fruita, and Palisade.
Schedule:
- Flexible shifts available Monday through Sunday
- Design your own schedule
- Minimum weekly or bi-weekly hourly commitment required
Key Responsibilities:
- Perform comprehensive annual home health risk assessments for Medicare Advantage plan members during 45-minute home visits
- Assess patients prior/current medical and medication histories
- Educate patients on diagnosed conditions, treatments, and medications
- Identify care gaps and recommend actionable steps for improved patient outcomes
- Visits may include annual assessments and post-discharge evaluations
- No prescribing or treatment plan creation required
Qualifications:
- Active Nurse Practitioner license in Colorado
- Board certified via AANP or ANCC
- Current BLS/CPR certification
- MSN degree from an accredited nursing school
- Minimum of 6 months recent Nurse Practitioner experience preferred
This position provides competitive compensationoffering $80-$90 per completed evaluation, along with a mileage stipend and pay for administrative time.
