An exciting opportunity is available for experienced Nurse Practitioners who are skilled in home health care to conduct home health risk assessments. This per diem position focuses on patient-centered care for Medicare Advantage plan members residing in Monroe County, AR, including Brinkley, Clarendon, Holly Grove, and surrounding areas. The role involves traveling to members' homes to perform risk assessments, each typically lasting about 45 minutes. The schedule is extremely flexiblechoose your own shifts, Monday through Sunday, with minimum weekly or bi-weekly hourly commitments tailored to your availability.

Key Responsibilities:
- Conduct comprehensive annual home health risk assessments in patient homes
- Evaluate prior and current medical history, medical conditions, and medications
- Educate patients on their health conditions and medication regimens
- Identify care gaps and provide recommendations to address these issues
- No prescribing or creation of treatment plans is required
Qualifications:
- Active Nurse Practitioner license in Arkansas
- Current CPR (BLS) certification
- Board certification through AANP or ANCC
- Minimum of 6 months recent experience as a Nurse Practitioner (preferred)
- MSN from an accredited nursing program
