Advance your career as a Nurse Practitioner by providing essential care to patients in their own homes throughout Putnam County. This per diem opportunity allows you to tailor your work schedule to suit your lifestyle, with flexibility to choose shifts any day of the week. Youll play a pivotal role in enhancing patient outcomes by conducting thorough risk assessments for Medicare Advantage plan members residing in Palatka and neighboring communities such as Interlachen and Crescent City.
Schedule:
- Flexible shifts available Monday through Sunday
- Create your own schedule to fit your needs
- Minimum hourly commitment required on a weekly or bi-weekly basis
Key Responsibilities:
- Perform comprehensive annual home health risk assessments, typically lasting 45 minutes per visit
- Travel to members residences across various locations within Putnam County
- Review and evaluate medical history, chronic conditions, and medication usage
- Educate patients on their medical conditions and proper medication usage
- Identify gaps in care and make recommendations to help address those gaps
- No need to develop treatment plans or prescribe medications
Qualifications:
- Active Nurse Practitioner license in Florida
- Board Certification (AANP or ANCC)
- Current BLS (CPR) certification
- MSN from an accredited nursing program
- At least 6 months of recent experience as a Nurse Practitioner preferred
Youll be compensated between $80 and $90 per completed evaluation, with additional mileage stipend and payment for any necessary administrative time.
