Job Description
Make a positive impact on patient care while enjoying the autonomy and flexibility of per diem work as a Nurse Practitioner delivering home health risk assessments across McHenry County, Illinois. In this role, you will conduct in-depth assessments for Medicare Advantage plan members in their homes, supporting both preventive care and post-discharge evaluations.
Schedule:

- Choose a minimum weekly or bi-weekly time commitment
Responsibilities:
- Perform comprehensive annual risk assessments for Medicare Advantage plan enrollees in communities including Crystal Lake, Algonquin, and Lake in the Hills
- Review prior and current medical history, health conditions, and medication regimens
- Provide patients with education about their existing medical issues and prescriptions
- Identify any unmet healthcare needs or care gaps and make appropriate recommendations for follow-up
- Focus solely on assessmentno prescribing or treatment plan development required
Qualifications:
- Active Illinois Nurse Practitioner license
- Current BLS (CPR) certification
- Board certified through AANP or ANCC
- MSN from an accredited nursing program
- At least 6 months of current Nurse Practitioner experience preferred
