Job Description

Step into a vital home health role providing comprehensive risk assessments for Medicare Advantage plan members across Steuben County, NY. This per diem opportunity allows Nurse Practitioners the flexibility to design their own schedules, conducting important assessments in a variety of community settingsincluding residences in Bath, Corning, Gang Mills, Hornell, and other local areas. Each visit, taking roughly 45 minutes, may encompass annual health evaluations or post-discharge reviews, contributing valuable insights into patients ongoing well-being.

Schedule:

  • Flexible shifts available seven days a week
  • Create a personalized work schedule
  • Minimum hourly commitment required on a weekly or bi-weekly basis

Key Responsibilities:

  • Perform in-home annual risk assessments for Medicare Advantage members
  • Review medical histories, current conditions, and medication usage
  • Provide patient education regarding health conditions and medication regimens
  • Identify care gaps and suggest actionable solutions for improvement
  • No medication prescribing or treatment plan development needed

Candidate Qualifications:

  • Current Nurse Practitioner license in New York State
  • Board Certification via AANP or ANCC
  • Valid CPR (BLS) certification
  • At least 6 months of recent Nurse Practitioner experience preferred
  • Master of Science in Nursing (MSN) from an accredited institution
