Job Description

Deliver high-quality care and independent assessments as a Nurse Practitioner providing in-home health risk evaluations across Steuben County, NY. This per diem opportunity allows experienced Nurse Practitioners to provide essential Medicare Advantage risk assessments in a flexible, patient-focused environment. Each assessment is approximately 45 minutes, encompassing general health evaluations or post-discharge visits. Locations may include Bath, Corning, Gang Mills, Hornell, and other areas within the county, with your shift assignments varying as needed.

Schedule:

  • Flexible shifts available Monday through Sunday
  • Set your own schedule around personal and professional commitments
  • Minimum weekly or bi-weekly hourly commitment

Key Responsibilities:

  • Perform comprehensive home health risk assessments for Medicare Advantage plan members
  • Review and evaluate medical history, current health status, and medication usage
  • Educate patients on medical conditions and medications
  • Identify care gaps and provide recommendations to address them
  • No requirement to create treatment plans or prescribe medication

Qualifications:

  • Licensed Nurse Practitioner in New York State
  • Active CPR (BLS) certification
  • Board Certification via AANP or ANCC
  • Minimum of six months current experience as an NP preferred
  • MSN from an accredited school of nursing
