Job Description

An exciting opportunity is available for an experienced Nurse Practitioner to provide home health risk assessments throughout Chemung County, NY. This role offers per diem, highly flexible work, allowing you to create your own schedule with shifts available seven days a week. Schedule:

Key Responsibilities:

  • Complete annual home health risk assessments for Medicare Advantage health plan members
  • Perform in-home evaluations of patient medical history, conditions, and medications
  • Educate individuals on their health conditions and prescribed medications
  • Identify gaps in care and provide appropriate recommendations to improve health outcomes
  • Visits may take place following hospitalization or as part of a scheduled annual assessment
  • No prescribing or creation of treatment plans requiredfocus is on assessment and education

Qualifications:

  • Active Nurse Practitioner license in New York State
  • Current CPR certification (BLS required)
  • Board certified by AANP or ANCC
  • Six months or more of recent Nurse Practitioner experience preferred
  • MSN degree from an accredited nursing program
