Embrace a dynamic and meaningful healthcare role as a Nurse Practitioner performing home health risk assessments on a per diem basis. In this position, you'll serve Medicare Advantage plan members across Chemung County, NY, including Elmira, Horseheads, Elmira Heights, and Wellsburg. Your day may involve an annual health evaluation or a post-discharge assessmenteach lasting approximately 45 minutesbringing personalized care directly to patients in their own homes.

- Choose a minimum weekly or bi-weekly hour commitment
Key Responsibilities:
- Conduct annual home health risk assessments for Medicare Advantage members
- Review and evaluate medical history, current conditions, and medication usage
- Provide clear education to patients regarding their health status and medications
- Identify gaps in care and recommend actionable next steps for improvement
- No requirement for treatment planning or prescribing medications
Qualifications:
- Active Nurse Practitioner license in New York State
- Current BLS (CPR) certification
- Board Certification via AANP or ANCC
- At least six months of Nurse Practitioner experience preferred
- MSN degree from an accredited program
