Make a difference with your clinical expertise by providing in-home risk assessments for Medicare Advantage members throughout Hendry County, FL. This per diem opportunity is ideal for Nurse Practitioners who excel in independent, patient-centered work and value flexible scheduling.
Schedule:
- Flexible shifts available Monday through Sunday
- Self-schedulingdesign your own workweek
- Minimal hourly commitment required (weekly or bi-weekly)
Key Responsibilities:
- Complete comprehensive, 45-minute in-home health risk assessments for Medicare Advantage plan members residing in Clewiston, LaBelle, Montura, and other Hendry County communities
- Evaluate patients prior and current medical histories, chronic conditions, and medication usage
- Deliver patient education on medical conditions and proper medication management
- Identify gaps in care and recommend next steps to optimize patient outcomes
- No ongoing treatment planning or medication prescribing involved
Qualifications:
- Active Nurse Practitioner license in Florida
- Board Certification through AANP or ANCC
- Master of Science in Nursing (MSN) from an accredited institution
- Current Basic Life Support (BLS) certification
- Minimum six months of NP experience preferred
- Experience in home health or community-based settings is an asset
