This per diem opportunity offers flexibility and autonomy for experienced nurse practitioners interested in conducting home health risk assessments for Medicare Advantage plan members throughout Preble County, Ohio. Visit patients in a variety of settingsincluding Camden, Lewisburg, and West Alexandriawhile enjoying the freedom to create your own schedule.
Schedule:
- Flexible shiftschoose any day, Monday through Sunday
- Design your own schedule
- Minimum weekly or bi-weekly hourly commitment
Responsibilities:
- Complete annual risk assessments and post-discharge visits in patient homes
- Evaluate and document prior/current medical history, health conditions, and medication usage
- Provide patient education about diagnoses, treatment, and medications
- Identify care gaps and advise on solutions to improve patient outcomes
- No requirement for prescribing medications or developing treatment plans
Qualifications:
- Active Nurse Practitioner license in Ohio
- Board Certification via AANP or ANCC
- Current CPR certification (BLS required)
- MSN degree from an accredited nursing school
- At least 6 months of current NP experience is preferred
This per diem position compensates $80$90 for each evaluation, along with a mileage stipend and compensation for administrative time.
