Job Description

Experience autonomy and flexibility as a Nurse Practitioner providing home health risk assessments on a per diem basis. This assignment involves conducting thorough in-home assessments for Medicare Advantage members across Vance County, including locations such as Henderson, Kittrell, and Middleburg. Each assessment typically lasts about 45 minutes and may consist of annual health checks or post-discharge visits. Your evaluations will support ongoing care management and help address gaps in patient care, without the requirement for treatment planning or medication prescribing.

Schedule:

  • Flexible shifts offered Monday through Sunday
  • Opportunity to create your own schedule
  • Minimum weekly or bi-weekly hourly commitment required

Key Responsibilities:

  • Conduct comprehensive annual home health risk assessments for Medicare Advantage plan members
  • Review prior and current medical histories, conditions, and medications
  • Educate patients regarding health conditions and medication usage
  • Identify and communicate gaps in care, recommending necessary follow-up for optimal health outcomes
  • Focus exclusively on assessmentno prescribing or creation of treatment plans

Qualifications:

  • Active Nurse Practitioner license in North Carolina
  • Board certificationAANP or ANCC required
  • Current CPR certification (BLS)
  • Master of Science in Nursing (MSN) from an accredited institution
  • At least six months of recent nurse practitioner experience preferred
