An exciting per diem opportunity is available for experienced Nurse Practitioners specializing in home health risk assessments. In this role, you will visit Medicare Advantage members throughout Daviess County, conducting comprehensive health evaluations right in their homes. Locations may include Owensboro, Whitesville, and Maceo, with each visit averaging about 45 minutes. The nature of each assessment varies, ranging from annual check-ups to post-discharge follow-ups.
Schedule:
- Flexible shifts between Monday and Sunday
- Self-scheduling to suit your availability
- Minimum commitment required on a weekly or bi-weekly basis
Key Responsibilities:
- Complete thorough annual risk assessments for Medicare Advantage members in their homes
- Review prior and current medical history, ongoing conditions, and medications
- Educate patients on their health conditions and medication regimen
- Identify care gaps and offer recommendations to address them
- No requirement to develop treatment plans or prescribe medications
Qualifications:
- Active Nurse Practitioner license in Kentucky
- Master of Science in Nursing (MSN) from an accredited institution
- Board certification through AANP or ANCC
- Current CPR certification (BLS)
- At least 6 months of recent Nurse Practitioner experience preferred
