Job Description

Deliver quality care in a flexible, per diem Nurse Practitioner position focused on home health risk assessments. In this role, you will travel to homes throughout Tioga County, conducting evaluations for Medicare Advantage plan members. Home visits are scheduled in areas including Waverly, Owego, and Newark Valley, with each risk assessment typically requiring about 45 minutes. Your assessments may include annual health evaluations or post-discharge visits, ensuring a diverse and rewarding scope of practice.

Schedule:

  • Flexible scheduling options between Monday and Sunday
  • Ability to design your own work schedule
  • Minimal weekly or bi-weekly commitment required

Key Responsibilities:

  • Complete comprehensive home health risk assessments for Medicare Advantage members
  • Review and document detailed medical histories, current conditions, and medication usage
  • Provide patient education on chronic conditions and prescribed medications
  • Detect and report any gaps in patient care; offer actionable recommendations
  • No responsibility for treatment plan development or medication prescription

Qualifications:

  • Active Nurse Practitioner license in the state of New York
  • Current BLS/CPR certification
  • Board certification (AANP or ANCC) is required
  • Minimum of 6 months experience as a practicing NP preferred
  • Master of Science in Nursing (MSN) degree from an accredited institution

In this position, you will earn $80 to $90 for each evaluation completed, along with mileage reimbursement and compensation for administrative time.
