Job Description
Seeking a dedicated healthcare professional to conduct home health risk assessments for Medicare Advantage members in Tioga County, NY. This per diem role is ideal for Nurse Practitioners who are passionate about improving patient outcomes through comprehensive, in-home evaluations and who value flexibility in their schedules. Assignments may take place in Waverly, Owego, Newark Valley, and other locations across Tioga County; candidates should be comfortable traveling within these areas.
Schedule:
- Flexible shifts available Monday through Sunday
- Set your own schedule and manage your availability
- Minimum weekly or bi-weekly hourly commitment
Key Responsibilities:
- Complete annual in-home risk assessments, averaging 45 minutes per visit, for Medicare Advantage plan members
- Assess and document current and prior medical history, conditions, and medication usage
- Provide focused health education to patients regarding medications and chronic conditions
- Identify and address care gaps, making appropriate recommendations for follow-up
- Note that prescribing medications and creating long-term treatment plans are not required
Preferred Qualifications:
- Active Nurse Practitioner license in the state of NY
- Board Certification via AANP or ANCC
- Current BLS (CPR) certification
- Master of Science in Nursing (MSN) from an accredited institution
- At least 6 months of recent Nurse Practitioner experience (strongly preferred)
In addition to a competitive rate of $80$90 per evaluation, practitioners receive a mileage stipend and compensation for administrative time.
