Job Description

As a Nurse Practitioner specializing in home health, you will play a crucial role in the healthcare experience of Medicare Advantage plan members by conducting comprehensive home-based risk assessments. This per diem opportunity offers exceptional scheduling flexibility, with shifts available every day of the week and the ability to tailor your schedule to your personal needs. Assignments are available throughout various residential communities in Seminole County, including areas such as Sanford, Altamonte Springs, and Casselberry.

Schedule:

  • Flexible shifts available Monday through Sunday
  • Set your own schedule
  • Minimum weekly or bi-weekly hourly commitment

Key Responsibilities:

  • Perform annual home health risk assessments for Medicare Advantage members in the comfort of their homes
  • Review and evaluate prior/current medical and medication history
  • Provide patient education on medical conditions and appropriate medication usage
  • Identify gaps in care, making informed recommendations to support better health outcomes
  • No treatment planning or prescription writing required

Qualifications:

  • Active Nurse Practitioner license in Florida
  • Board Certification via AANP or ANCC
  • Graduation from an accredited MSN program
  • Current CPR certification (BLS)
  • At least 6 months of recent NP experience preferred

Nurse Practitioners in this role are compensated $80$90 per completed evaluation, plus additional stipends for mileage and administrative duties.
