Step into a dynamic per diem opportunity where your expertise as a Nurse Practitioner supports quality patient care in the home health setting. In this role, youll perform comprehensive home health risk assessments for Medicare Advantage plan members across various neighborhoods in Volusia County. Each visit, typically lasting about 45 minutes, may entail annual wellness assessments or post-discharge evaluations, fostering continuity and preventive care for seniors.
Schedule/Hours:
- Enjoy flexible shifts available any day between Monday and Sunday
- Design your own schedule
- Commit to a minimum weekly or bi-weekly number of hours based on your availability
Key Responsibilities:
- Complete annual risk assessments in patient homes
- Review prior and current medical history, existing conditions, and medication use
- Educate patients on diagnoses, wellness, and medication adherence
- Identify gaps in care and share actionable recommendations
- No responsibility for developing treatment plans or prescribing medications
Qualifications:
- Active Nurse Practitioner license in Florida
- Current BLS/CPR certification
- Board certification from AANP or ANCC
- At least 6 months of current NP experience preferred
- MSN from an accredited school of nursing
This position offers competitive compensation with payment of $80-$90 per evaluation, including a mileage stipend and compensation for administrative time.
