Advance your career providing meaningful care directly in patients homes as a Nurse Practitioner specializing in home health risk assessments. In this per diem opportunity, youll support Medicare Advantage members throughout Allen County, Ohioincluding Lima, American Township, and Shawnee Townshipdelivering preventative healthcare in a flexible, independent practice setting.

Schedule:

• Flexible shifts available Monday through Sunday
• Create your own work schedule
• Choose your preferred minimum weekly or bi-weekly hourly commitment

Key Responsibilities:

• Conduct annual home health risk assessments for Medicare Advantage members, typically lasting 45 minutes per visit
• Evaluate patients medical history, current conditions, and medication usage
• Educate patients and families about health conditions and appropriate medication usage
• Identify gaps in care, highlighting opportunities for intervention and recommending resolution strategies
• Visits may include annual assessments or post-discharge evaluations
• This role does not require prescribing medications or developing treatment plans

Qualifications:

• Active Nurse Practitioner License in Ohio
• Current CPR certification (BLS)
• Board certification through AANP or ANCC
• MSN degree from an accredited nursing school
• At least 6 months of experience as a Nurse Practitioner preferred
