Job Description

Advance your career as a Nurse Practitioner specializing in home health risk assessments. This per diem opportunity allows experienced NPs to provide valuable healthcare services throughout Oswego County, including areas such as Fulton, Brewerton, and Pulaski. You will visit Medicare Advantage plan members in their homes, conducting evaluations that play a crucial role in optimizing their ongoing care.

Schedule/Hours:

  • Flexible shifts available Monday through Sunday
  • Ability to create your own schedule
  • Minimum weekly or bi-weekly hourly commitment

Key Responsibilities:

  • Perform annual home health risk assessments for Medicare Advantage members
  • Review and assess medical history, ongoing conditions, and current medication regimens
  • Deliver patient education regarding existing health conditions and prescribed medications
  • Identify care gaps and offer meaningful recommendations for addressing them
  • Documentation as required; no need to formulate treatment plans or prescribe medications

Qualifications:

  • Active Nurse Practitioner License in New York
  • Certified in Basic Life Support (CPR/BLS)
  • Board certified through AANP or ANCC
  • Minimum 6 months of current NP experience preferred
  • Master of Science in Nursing (MSN) from an accredited institution
