Job Description

Experienced nurse practitioners are needed to provide per diem home health risk assessments for Medicare Advantage members throughout Oswego County. In this flexible, patient-focused role, you will visit members in their homesacross areas such as Fulton, Brewerton, and Pulaskito complete comprehensive evaluations. Each visit averages about 45 minutes and may include annual assessments or post-discharge check-ins.

Schedule:

  • Flexible shifts available seven days a week
  • Set your own schedule
  • Minimum weekly or bi-weekly hourly commitment

Key Responsibilities:

  • Perform annual or post-discharge risk assessments in patients homes, focusing on their medical and medication histories
  • Review prior and current medical conditions, identifying gaps in care and recommending solutions
  • Educate patients on health conditions and medications to promote understanding and compliance
  • Document findings for the Medicare Advantage plan
  • No requirement to create treatment plans or prescribe medications

Qualifications:

  • Active Nurse Practitioner license in New York
  • Board certification through AANP or ANCC
  • Current CPR (BLS) certification
  • MSN degree from an accredited school of nursing
  • At least 6 months of recent NP experience preferred
