Exciting opportunity for Nurse Practitioners passionate about patient-centered care in a home health setting. In this per diem role, youll travel throughout Oswego Countyvisiting patients in areas including Fulton, Brewerton, and Pulaskito conduct important home health risk assessments for Medicare Advantage plan members.
Schedule:
- Flexible shifts available Monday through Sunday

- Minimum hourly commitment per week or every two weeks, tailored to your preferences
Key Responsibilities:
- Conduct comprehensive annual home health risk assessments in members homes
- Review prior and current medical histories, ongoing conditions, and medication use
- Deliver easy-to-understand education about diagnoses and prescribed medications
- Identify potential gaps in care and recommend practical solutions for better health outcomes
- No prescribing or creation of individual treatment plans required
Qualifications:
- Active Nurse Practitioner license in the state of New York
- Current BLS CPR certification
- Board certification as an NP through AANP or ANCC
- At least 6 months of current experience as a Nurse Practitioner is preferred
- MSN degree from an accredited nursing program
