Elevate patient care as a Nurse Practitioner specializing in home health risk assessments for Medicare Advantage plan members across Adams County, IN. This per diem opportunity offers exceptional flexibility, allowing you to create your own schedule and select shifts between Monday and Sunday. Assignments may include visits in Decatur, Berne, Geneva, and surrounding areas. Each assessment averages 45 minutes and may be annual health checks or post-discharge visits, with all necessary resources provided.
Schedule:
- Flexible shifts available Monday through Sunday
- Create your own schedule
- Minimum weekly or bi-weekly hourly commitment
Key Responsibilities:
- Conduct one-on-one annual health risk assessments in members homes
- Review past and current medical histories, diagnoses, and medications
- Deliver clear education on medical conditions and pharmaceutical regimens
- Identify care gaps and offer recommendations for improved health outcomes
- No requirement to create full treatment plans or prescribe medications
Qualifications:
- Active Nurse Practitioner license in Indiana (IN)
- Current BLS/CPR certification
- Board Certification through AANP or ANCC
- At least 6 months of recent NP experience preferred
- Master of Science in Nursing (MSN) from an accredited program
