Deliver comprehensive care in a dynamic per diem role conducting home health risk assessments for Medicare Advantage members across Shoshone County, ID. Key Responsibilities:

• Perform detailed annual home health risk assessments and focused post-discharge evaluations
• Assess medical histories, ongoing conditions, and complete medication reviews
• Offer patient education on current health conditions and ensure understanding of prescribed medications
• Detect gaps in care and recommend appropriate interventions to enhance health outcomes
• No prescriptive authority or treatment plan management requiredemphasis on assessment, education, and gap closure

Qualifications:

• Active Nurse Practitioner license in Idaho
• Valid CPR certification (BLS required)
• Board certified via AANP or ANCC
• MSN from an accredited nursing school
• At least six months of current Nurse Practitioner experience preferred
