Job Description

Advance your practice as a Nurse Practitioner specializing in Home Health by performing focused risk assessments throughout Shoshone County, ID. This per diem opportunity offers significant autonomy and flexibilityschedule your own shifts and adapt your hours to fit your lifestyle, with visits possible in Kellogg, Pinehurst, Osburn, and surrounding areas.

Schedule:

  • Flexible shifts available any day, Monday through Sunday
  • Create your own schedule
  • Meet minimum weekly or bi-weekly hourly commitments

Key Responsibilities:

  • Complete comprehensive home health risk assessments for Medicare Advantage memberseach taking approximately 45 minutes
  • Review prior and current medical history, evaluate medical conditions, and assess medication use
  • Educate patients regarding their medical conditions and medications
  • Identify care gaps and offer recommendations, supporting care continuity and improved outcomes
  • No diagnosis, prescription, or treatment planning is requiredfocus on assessment and patient education

Qualifications:

  • Nurse Practitioner licenseactive in the state of Idaho
  • Current BLS (CPR) certification
  • Board Certified through AANP or ANCC
  • MSN from an accredited school of nursing
  • At least six months of recent Nurse Practitioner experience preferred
