Job Description

Make a meaningful impact as an Advanced Practice Provider specializing in home health by performing comprehensive risk assessments for Medicare Advantage plan members. Working per diem, you'll visit patients in their homes throughout Seminole County, enabling vital care delivery and supporting better health outcomes while benefiting from flexible scheduling.

Schedule:

  • Shifts available any day of the week, Monday through Sunday
  • Self-scheduling: set your own hours
  • Options for weekly or bi-weekly hourly commitments

Key Responsibilities:

  • Perform detailed annual health risk assessments during home visits for Medicare Advantage members
  • Review medical history, current health status, and medication use to gain a complete clinical picture
  • Provide patient education related to diagnoses, medications, and wellness strategies
  • Identify and document gaps in care, suggesting ways to address each area of concern
  • No need for creating treatment plans or prescribing medications

Qualifications:

  • Active Nurse Practitioner license in Florida
  • MSN degree from an accredited nursing program
  • Board Certification as a Nurse Practitioner (AANP or ANCC)
  • Valid CPR certification (BLS)
  • At least 6 months of current Nurse Practitioner experience preferred

These home health visits average 45 minutes each and take place in communities such as Sanford, Altamonte Springs, and Casselberry, among others within Seminole County. Assignments may vary by shift, offering diversity in both patients and locations.

In addition to earning $80$90 per completed evaluation, youll receive stipends for mileage and administrative time, ensuring your professional commitment is recognized and rewarded.
