Job Description

This per diem opportunity offers exceptional flexibilitychoose your own schedule with shifts available seven days a week and a minimum hourly commitment to fit your lifestyle.

Schedule:

  • Flexible shifts between Monday and Sunday
  • Create your own schedule
  • Minimum weekly or bi-weekly hourly commitment

Responsibilities:

  • Conduct annual health risk assessments in members' residences; each visit typically lasts approximately 45 minutes.
  • Visits may include routine annual assessments or post-discharge evaluations.
  • Review medical history, current conditions, and medications.
  • Educate patients on their health conditions, medication management, and wellness strategies.
  • Identify and document any care gaps, providing actionable recommendations to improve patient outcomes.
  • No requirement to develop treatment plans or prescribe medications.

Qualifications:

  • Active Nurse Practitioner license in Florida.
  • Current CPR certification (BLS).
  • Board Certification through AANP or ANCC.
  • MSN degree from an accredited nursing program.
  • A minimum of 6 months of recent NP experience is preferred.
  • Excellent interpersonal and clinical assessment skills.
