Job Description
Deliver comprehensive healthcare to senior living and homebound insurance members in Oregon City, OR, focusing on preventing unnecessary hospitalizations through in-place treatment. This contract role requires regular health assessmentsat least monthly, and more frequently as neededto monitor patient status, provide care, and offer resources that reduce emergency room visits and hospital stays.
Schedule:
- Monday through Friday
- 8 AM to 5 PM
- No weekends or holidays
Key Responsibilities:
- Develop detailed problem lists and individualized medical treatment plans
- Provide primary, preventative, urgent, and acute care directly on-site
- Identify and stratify high-risk patients for increased clinical attention
- Create contingency plans tailored to high-risk conditions
- Collaborate closely with residents primary care physicians to ensure continuity of care
- Deliver hands-on bedside care, craft care plans, and generate medical orders
- Partner with facility staff and Directors of Nursing (DON) in senior living communities
- Conduct single visits for patients recently discharged to home from skilled nursing facilities (SNF)
Qualifications:
- Master of Science in Nursing (MSN) degree
- Valid Oregon Nurse Practitioner (NP) license
- Basic Life Support (BLS) certification
- Drug Enforcement Administration (DEA) registration
- Preferred experience in home health, hospice, or related areas
