Job Description
Make a difference by delivering high-quality, patient-centered care in a flexible per diem Nurse Practitioner role focused on home health risk assessments. This position serves Medicare Advantage plan members across Otero County, ensuring valuable assessments for individuals in La Junta, Rocky Ford, Fowler, and nearby areas.
Schedule:
- Flexible shifts available Monday through Sunday
- Create your own schedule to fit your availability
- Minimum commitment required weekly or bi-weekly
Key Responsibilities:
- Conduct annual risk assessments in-home for Medicare Advantage members
- Review and evaluate current and prior medical history, conditions, and medications
- Educate members regarding their conditions and prescribed medications
- Identify gaps in care, offering recommendations and resources for improved health outcomes
- No responsibility for creating treatment plans or prescribing medications
Qualifications:
- Active Nurse Practitioner license in Colorado
- Current CPR certification (BLS required)
- Board certification through AANP or ANCC
- MSN from an accredited nursing program
- Minimum of 6 months Nurse Practitioner experience preferred
Compensation for this role ranges from $80-$90 per evaluation, which also includes a mileage stipend for travel between visits and compensation for administrative time.
