Make an impact by delivering exceptional care in the homes of Medicare Advantage members throughout diverse communities. As a Nurse Practitioner specializing in home health visits on a per diem basis, youll play a pivotal role in improving patient outcomes while enjoying flexibility and autonomy in your schedule.
Schedule/Hours
- Flexible schedule: shifts available Monday through Sunday
- Design your own work schedule
- Minimum weekly or bi-weekly hourly commitment required
Key Responsibilities
- Conduct comprehensive home health risk assessments for plan members across Duval County, including annual assessments and post-discharge visits
- Review and evaluate each patients prior and current medical history, medical conditions, and medication history
- Provide patient education regarding medical conditions and prescribed medications
- Identify care gaps and offer recommendations to address them
- No requirements for treatment planning or medication prescribing
Desired Qualifications
- Current and unrestricted Nurse Practitioner license in Florida
- Board Certification from AANP or ANCC
- MSN degree from an accredited institution
- Valid CPR certification (BLS)
- At least six months of experience as a Nurse Practitioner preferred
This per diem opportunity offers a competitive rate of $80 to $90 per completed evaluation, along with a mileage stipend and compensation for administrative tasks.
