Deliver valuable care in the community by performing home health risk assessments for Medicare Advantage plan members throughout Miami-Dade County. In this rewarding per diem opportunity, youll have the chance to make a meaningful impact while enjoying flexibility and autonomy.
Schedule:
- Create your own scheduleset shifts that suit your availability between Monday and Sunday
- Choose minimum weekly or bi-weekly hourly commitments
Key Responsibilities:
- Complete in-home health risk assessments (average 45 minutes each) for Medicare Advantage members in varied locations, such as Miami, Hialeah, and Miami Gardens
- Review and evaluate patients prior and current medical histories, conditions, and medication usage
- Provide education to patients regarding health conditions and prescribed medications
- Identify care gaps, sharing recommendations to resolve them as needed
- Focus exclusively on assessmentno responsibility for prescribing medications or developing treatment plans
Required Qualifications:
- Nurse Practitioner license in the state of Florida
- Active Board Certification (AANP or ANCC)
- Master of Science in Nursing (MSN) from an accredited nursing program
- Current Basic Life Support (BLS) CPR certification
- At least 6 months of recent Nurse Practitioner experience preferred
