Take on a dynamic per diem role as a Nurse Practitioner, conducting home health risk assessments for Medicare Advantage plan members throughout Hamilton County, Tennessee. This unique position offers the flexibility to design your own work schedule, choosing shifts between Monday and Sunday with a minimum weekly or bi-weekly commitment, allowing you to balance your professional and personal priorities seamlessly.

Key Responsibilities:
- Perform comprehensive annual health risk assessments in members homes
- Review and evaluate medical histories, existing health conditions, and medication usage during each visit (approximately 45 minutes per assessment)
- Educate patients about their diagnoses and prescribed medications
- Recognize care gaps and provide actionable recommendations to address them
- Focus exclusively on assessment; no prescription writing or ongoing treatment planning necessary
Qualifications:
- Active Nurse Practitioner License in Tennessee
- Board Certification by AANP or ANCC
- Current BLS CPR certification
- MSN from an accredited nursing school
- Minimum of 6 months recent Nurse Practitioner experience (preferred)
This position allows you to make a significant impact by identifying and addressing health needs directly in patients homes in various neighborhoods, including Chattanooga, East Ridge, Soddy-Daisy, and surrounding communities. Compensation includes $80-$90 per completed evaluation, plus a mileage stipend and administrative time.
