Play a vital role in supporting community health by performing in-home risk assessments for Medicare Advantage plan members across Hamilton County, TN. This per diem opportunity is designed for Nurse Practitioners passionate about engaging with patients in their own environment, providing valuable preventive care and education. Assessments are typically straightforward, lasting about 45 minutes, with visits potentially covering annual screenings or post-discharge evaluations.

- Meet a minimum weekly or bi-weekly hourly commitment
Key Responsibilities:
- Complete annual home health risk assessments for Medicare Advantage members
- Thoroughly evaluate patients medical histories, ongoing conditions, and medications
- Educate patients regarding their medical conditions and medication adherence
- Identify care gaps and make recommendations to foster improved patient outcomes
- Focus strictly on assessmentno requirement for treatment plan development or prescription management
Qualifications:
- Active Nurse Practitioner license in Tennessee
- Current BLS/CPR certification
- National Board Certification (AANP or ANCC)
- At least 6 months of recent NP experience preferred
- Master of Science in Nursing (MSN) from an accredited institution
