Experience the dynamic field of home health as a Nurse Practitioner conducting comprehensive risk assessments in Boone County, Arkansas. This per diem position allows Nurse Practitioners to leverage their clinical expertise to improve outcomes for Medicare Advantage members in a flexible, patient-centered setting.

Schedule:

• Flexibility to work shifts between Monday and Sunday
• Design your own work schedule
• Meet minimum weekly or bi-weekly hour commitments tailored to your availability

Key Responsibilities:

• Complete annual or post-discharge home health risk assessments for Medicare Advantage enrollees across locations such as Harrison, Diamond City, and Bergman
• Review and document patients prior and present medical conditions, history, and medication usage
• Deliver clear education about health conditions and medication usage to patients and caregivers
• Identify care gaps and make informed recommendations to promote health and well-being
• No responsibility for developing treatment plans or prescribing medications

Qualifications:

• Active Nurse Practitioner License in Arkansas
• Board certification (AANP or ANCC required)
• Current CPR (BLS) certification
• MSN from an accredited nursing program
• Minimum 6 months of nurse practitioner experience is preferred

Nurse Practitioners in this role are compensated $80$90 per completed evaluation, plus a mileage stipend and additional administrative time.
