Schedule:
- Flexible shifts available Monday through Sunday
- Create your own schedule
- Satisfy a minimum weekly or bi-weekly hourly commitment
Responsibilities:
- Conduct thorough home health risk assessments for Medicare Advantage plan members
- Average visit time is approximately 45 minutes per assessment, with assignments ranging from annual health reviews to post-discharge check-ins
- Gather comprehensive medical information, including medical history, current conditions, and medication lists
- Educate members on health conditions, medication compliance, and overall wellness strategies
- Identify gaps in patient care and offer recommendations to support improved outcomes
- No medication prescribing or treatment plan development is necessary; focus is on assessment and education
Qualifications:
- Active Nurse Practitioner license in NJ
- Current CPR (BLS) certification
- Board certified as an NP through AANP or ANCC
- Masters degree in Nursing (MSN) from an accredited institution
- At least 6 months of recent NP experience strongly preferred
This position offers highly competitive compensation$80$90 per completed evaluation, in addition to a mileage stipend and paid administrative time. Work in highly desirable areas throughout Monmouth County, including Middletown, Howell, and Marlboro, with each shift assigning visits in these communities.
