Deliver high-quality patient care in a meaningful setting as a Nurse Practitioner specializing in home health risk assessments. Youll play a vital role in supporting Medicare Advantage plan members by conducting comprehensive home visits across Montrose County, which includes communities such as Montrose and Naturita. Every assessment offers the opportunity to make a direct impact by improving health outcomes for seniors in their own environments.

Key Responsibilities:

• Perform thorough annual health risk assessments for Medicare Advantage members in their homes
• Review and evaluate prior/current medical history, ongoing medical conditions, and current medication regimens
• Educate patients on their health conditions and prescribed medications to enhance self-management
• Identify care gaps and provide actionable recommendations for follow-up
• No requirement to prescribe medications or create new treatment plans

Qualifications:

• Unrestricted Nurse Practitioner license in Colorado
• Current BLS (CPR) certification
• National board certification through AANP or ANCC
• At least 6 months of recent NP experience preferred
• MSN from an accredited nursing program
