Job Description

A rewarding per diem opportunity is available for an experienced Nurse Practitioner to perform home health risk assessments for Medicare Advantage plan members across Montrose County, CO. This highly flexible position allows you to structure your schedule, with shifts available any day of the week and a minimum weekly or bi-weekly time commitment.

Schedule:

  • Flexible shifts Monday through Sunday
  • Create your own schedule
  • Meet minimum weekly or bi-weekly hours

Key Responsibilities:

  • Conduct home-based annual risk assessments for Medicare Advantage members in Montrose County, including areas such as Montrose and Naturita.
  • Spend approximately 45 minutes per risk assessment visit, which may be an initial annual assessment or a follow-up post-discharge visit.
  • Review and evaluate current and prior medical history, medical conditions, and medication use.
  • Provide patient education about medical conditions and medication management.
  • Identify gaps in care and offer actionable recommendations to close those gaps.
  • No responsibility for creating treatment plans or prescribing medications.

Qualifications:

  • Active Nurse Practitioner license in the state of Colorado
  • Current CPR certification (BLS)
  • Board certified through AANP or ANCC
  • Minimum of 6 months recent experience as a Nurse Practitioner preferred
  • Master of Science in Nursing (MSN) degree from an accredited nursing program

This position offers compensation for each completed evaluation, ranging from $80 to $90, plus a mileage stipend and administrative time.
